Although you are far from the exchange you should have at least 2mb.
Its connected at the right modulation (G992.5)
Its got "fast path" rather than "interleaved" on, fastpath shouldnt really be enabled on any lines >30dB, yours is 61.
Your output DbM power is shit, it should be 12 and 19, so you have most likely a line fault.
So, i'd start with getting an interleaved profile, it adds more CRC checks so basically takes your speed down a bit, but yours is already very low and it should make it more stable.
To confirm the line fault the next thing to do is plug your router into the BT Test socket (its found by removing the faceplate of the main BT Socket in the house) and sticking the router in there. You only need to do this quickly so its online for around 5mins and then paste the stats back in here again.
